**If you would like to become a medicaid waiver provider, please contact Roberta Phifer at (904)992-2426 or email roberta_phifer@apd.state.fl.us. Provider applications cannot be mailed out, but can be picked up at the Area 4 Agency for Persons with Disabilities Office, 3631 Hodges Blvd, Bldg A, Jacksonville, Fl 32224. Area 4 has a surplus of providers, but is seeking dentist for adults with developmental disabilities.

To apply for services from the Agency for Persons with Disabilities, including the Medicaid Waiver program, for yourself or a family member, please contact Ryan Murphy at (904)992-2428 or email ryan_murphy@apd.state.fl.us.
Services are not based on your income but are based on the developmental disabilities covered under the Florida States 393. "Developmental Disability" is a broad term that refers to a variety of conditions that interfere with a person's ability to function in everyday activities. Chapter 393, Florida Statutes, defines developmental disabilities as spina bifida, autism, cerebral palsy, Prader-Willi syndrome and mental retardation.
